Recall & Reminder System Policy
Practice Policy: Our practice is committed to ensuring that all tests, results, diagnostic imaging reports, investigation reports, and clinical correspondence are promptly reviewed, flagged with an action statement, and acted upon by the GP. These actions will be recorded in the patient health record. All tests and results, especially those requiring follow-up, are part of our Recall and Reminder system. This system is designed to provide continuity of care, better management of chronic conditions, and preventive care for improved patient outcomes.
Review Process:
- All results, reports, and correspondence are reviewed by the GP through our computer system (Best Practice Inbox).
- GPs will flag patients of concern and identify if action is needed, including urgent or routine follow-up.
- Patients are informed about our notification procedure for results and follow-ups through verbal communication during consultations and our practice information sheet.
Recall System Overview: Our practice uses a structured recall system to ensure patients are promptly contacted for necessary follow-ups or test result discussions. Recalls are an essential component of quality care, providing proactive health management and reducing the likelihood of acute health issues.
Types of Recalls:
- Urgent Recalls: Required within 24 hours, often for abnormal results or urgent referral responses. Reception staff is alerted to prioritise and immediately contact patients for same-day or next-day appointments.
- Non-Urgent Recalls: These are typically for follow-ups or tests that need to be addressed within 14 days, either through face-to-face or telephone consultation.
